Making an Appointment

Dear Valued Patient,

These are exciting times at Eisner Pediatric & Family Medical Center. To serve you better, we have installed an information system called the Electronic Health Record. It will help us to provide you with better medical care and give us easier access to your medical records. We will be more efficient and you will have fewer forms to fill out.

Our staff is taking special training on the new system. During this time, there will be some delays. There may be fewer appointments available and it may take a little longer to see your provider. We ask your patience and understanding.

To make the waiting go faster, please call the clinic if there has been a change in your phone number or address. It is important that we have your current information so that we may mail you an appointment card and give you a reminder call.
